2026 The Floor 4 Training:

A Foundational Approach to Airway and Whole-Body Healing

Unlock the Connection Between the Floor of the Mouth and the Entire Body.

The Floor 4 training is a specialized, two-day intensive workshop designed for interdisciplinary healers. It represents a distillation and crystallization of the Floor of the Mouth Tour, focusing on the powerful two-way street connection between oral health and systemic physical function.

Led by Dr. Neel Bulchandani, an expert with nearly 18 years of experience in healing and nine years specializing in airway and feeding, this course provides the tools to bring balance to the body by addressing the foundation of the oral-facial complex.

For some feedback from the Module 1 portion of the Los Angeles Floor of the Mouth Tour from March, watch the video from Mayzy, a LA-based Rolfer (formally-trained at the original Structural Integration institute).

What is the "Floor 4"?

The training simplifies the 70 critical areas of the airway map into four foundational floor areas that impact the entire body:

The Head: Addressing cranial tension and oral-facial posture.

The Throat: Focusing on airway space and the oral diaphragm.

The Sides: Balancing the lateral fascial chains and shoulders.

The Feet: Correcting downstream issues like arch height and stability that originate in the mouth.

Who Is This Training For?

This workshop is built for an interdisciplinary healer capacity, including:

Myofunctional Therapists (RDHs and SLPs)

Physical & Occupational Therapists (PTs and OTs)

Chiropractors & Osteopaths (DCs and DOs)

Bodyworkers: Massage therapists, Rolfers, and structural integrationists

Specialized Practitioners: Craniosacral therapists, and visceral workers

Note: Whether your scope of practice allows for hands-on treatment or you work in a virtual capacity, you will learn how to screen and guide patients through self-assessment protocols.

Clinical Applications & Benefits

The Floor 4 approach addresses a wide range of issues beyond just airway and feeding, including:

  • Fascial & Mobility Issues: Rigid fascia in the neck (SCM), suboccipital muscles, trapezius, and psoas.

  • Oral-Facial Challenges: Tongue-tie, lip-tie, narrow palates, and hypermobility of the jaw.

  • Systemic Symptoms: TMJ, headaches, swallowing issues, gag reflex hypersensitivity, and plantar fasciitis.

  • Pre- and Post-Surgical Support: Preparing patients for and supporting recovery from frenectomies, palate expansions, and maxillofacial surgeries like MIND or DOME.

Your Roadmap to Mastery

Over two days, you will move through a comprehensive system designed to accelerate your practice:

  • Assessment Protocol: Learn to identify restricted mobility and hypotonic muscle tone.

  • Treatment System: Master hands-on and guided release techniques for the four floor areas.

  • The Arc of Care: Move beyond one-time encounters to guide patients through a long-term system of healing and symmetry.

Who Is This For?

This is for anyone in health care currently working with patients with oral ties or those who desire to be involved in this rapidly growing field. Whether you’re a massage therapist, bodyworker, myofunctional therapist, physical therapist, chiropractor, naturopath, osteopath, dental professional, ENT professional, speech therapist, sensory-processing therapist, or anyone in the birth worker field (doulas, midwives, lactation consultants, obstetricians, nurses), this workshop can benefit you greatly. If you want to draft on Dr. Neel’s collaborative relationships with Tongue-Tie professionals (locally, statewide, and internationally), I highly recommend attending this workshop.

Is this only for hands-on therapists and practitioners?

No, this is for anyone interested in learning more about this field and desiring to be better positioned to discuss it with peers and patients. Certain portions will be geared towards those with specialized training in issues accompanying tongue ties — Craniosacral Therapists, Craniosacral Fascial Therapists/Gillespie Approach, myofunctional therapists, Rolfers, and chiropractors/osteopaths. For those hands-on practitioners, advanced techniques will be more readily grasped and ascertained and, of course, implemented; however, it’s advantageous for all involved in this field to observe healing modalities that can broaden and deepen our collective understanding in helping this seemingly-growing subset of the population.

How the technique originated

In his collaborative approach with Tongue-Tie providers in Redding (and in other areas of California) and through the process of having 3 children with oral ties, Dr. Neel has identified patterns that accompany patients with tongue ties — from those in cranial and spinal misalignments to those in body-wide myofascial (soft tissue) restrictions and even those related to nutritional deficiencies and environmental toxicities. Through hands-on care with many patients in the last 5 years and his research and cross-referral work with professionals in related fields of dentistry, otolaryngology, osteopathy, myofunctional therapy, and frequency medicine, Dr. Neel has systematized a protocol for facilitating the improvement of mobility and function in patients with oral ties.
